San Diego Unified School District's partnership with Hazel Health means your student can get high-quality telehealth care at no cost to your family. Students can get virtual medical care at school and at home on any smartphone, tablet, or computer. All Hazel Health visits are safe and confidential.

How telehealth with Hazel Health works

Telehealth is the use of technology to provide health care virtually. All Hazel visits are virtual visits.

You only need to complete the consent form one time for each student.

The first step to connect your student to care is completing our consent form. When you fill out the consent form, we will ask for the following details:

How teletherapy works

Intake visit

You and your student meet with a Hazel Intake Coordinator to provide information and create goals for teletherapy

Short-term teletherapy

The Hazel therapist works with you and your student to develop an evidence-based personalized program

Care beyond Hazel

If your student needs long-term care we will support them until they can see a local therapist

You can refer your student for therapy, here's how:

  1. Complete consent

  4. Within 2 business days, a Hazel Health representative will contact you to schedule an intake visit.

When your student visits the school health office, designated school staff use an iPad to connect them to a Hazel provider for care.

Medical care at home

When your student needs care from home, you can request a telehealth medical visit, which you and your student can then join on any computer or smart device.

After the visit

After the visit, Hazel recommends ongoing care if needed and follows up with the legal guardian.

How we partner with families

Parent permission

With your permission, your child will have access to Hazel’s care team. You only need to complete the consent form one time for each child.

Therapy referral

Referrals can come from you as the parent, or from trusted school staff who know your child.

Care at no cost to families

By partnering with your school, Hazel visits are at no cost to your family.

Visits from home

From home, your child can connect with a Hazel provider or join their scheduled therapy appointment.

Visit involvement

You can participate in your child’s visits virtually, from home or from work.

Continued support

After care, if needed, our clinical support team follows up with you to continue to support your child’s needs.

Telehealth is the use of technology to provide health care virtually. All Hazel visits are "virtual visits", through a tablet, desktop, or other smart device.

For the current school year, Hazel Health’s services are provided at no out-of-pocket cost to families.

When families give their consent and provide insurance information, Hazel Health can bill insurance, just like any other healthcare provider. This helps us get reimbursed for the services we provide to students, making sure that some or all of the visit cost is covered by insurance whenever possible. A portion of the fee that districts pay Hazel as a platform fee is used to offset any out-of-pocket expenses not covered by insurance.ᅠ

Once care has been provided, having access to insurance information also helps to better coordinate support of families, such as by sharing referral recommendations or ordering prescriptions, to apply a more integrated approach to physical and mental health care for students. Additionally, knowing which plans our families have helps us identify the health plans that Hazel Health should contract with so that the visit cost can be covered by insurance whenever possible.

If an insurance card is unavailable, the parent/guardian should complete as much information as possible on the consent form. The parent/guardian can call Hazel Health anytime with additional details if available.
